Basketball season continues in 2024! We will continue working on skills through fun, team-oriented drills. We will also be playing 5-on-5 games again with our players, and are bringing volunteer teams in as well. As always the emphasis is on having fun and being a good teammate!
This program is designed for athletes age 7-13
I do hereby assume full responsibility for any and all damages, injuries (including death), or losses that my child may sustain or incur, if any, while attending, practicing, participating, or witnessing in any program sport of physical activity occurring or about the premises or at any offside location. I hereby assume full risk, waive all claims and release and hold Empower Sports, its instructors, or partners of said program or event, individually or otherwise, harmless for any and all claims for injuries or damages.
I am fully aware and understand that Empower Sports does not have on or about the premises, or employ or contract with any medical services, provisions for ordinary or emergency medical services.
In consideration of my child’s participation in and the use of the facility, I hereby release and covenant not to sue Empower Sports, its owners, shareholders, directors, officers, employees, representatives, agents, volunteers, and lessees from any and all claims resulting from any physical injury that may occur to my child while participating in any program or event sponsored by Empower Sports.
The SIGNER grants permissions to Empower Sports to use the Signer’s child’s image, likeness, actions, and statements in any live or recorded audio, video, or photographic display or other transmission, exhibition, publication or reproduction made of or at the Event in any medium or content without further authorization or compensation
